Fully comprehensive, the text is supplemented by an impressive array of images and illustrations, which it uses to walk the reader through complex procedures with ease. It is up-to-date and advanced enough for even the most experienced practitioners, but fundamental enough to be used as an introductory text for trainees and students. Extremely well written with lots of fantastic illustrations and pictures that make it so easy to read.
Figures and illustrations are really helpful in understanding of techniques and anatomical structures. All technical approaches and interventions are presented in detail and information on the molecular mechanism of action is also provided based on the most recent scientific advances.
An excellent choice both for trainees and experts. A source of knowledge that practitioners can turn to for guidance and update.
Fully half the book's content is devoted to pain medicine, making it useful as an authoritative reference for physicians treating pain whether or not they practice regional anesthesia.
I definitely believe that it is a textbook that everyone interested in pain management and neural blockade should have.

Renal: chronic renal failure, child with a transplanted kidney, etc. CNS: seizures, brain tumors, cerebral palsy, mental retardation, etc.

Heme/Onc: anemia, leukemia, chemotherapy, mediastinal mass, hemophilia, sickle cell disease, etc.

Trauma: CNS, truncal, limb, etc.

Airway syndromes: Treacher-Collins, Goldenhars, cystic hygroma, etc.

Miscellaneous: Trisomy 21, epidermolysis bullosa, etc.

III. Preoperative Considerations

This chapter will discuss principles of premedication, psychological preparation, parents in the OR, appropriate preoperative labs, & principles of preoperative fasting.

IV. Intraoperative Monitoring & Management

This chapter will discuss monitors, airway equipment, and induction & maintenance of general anesthesia.

V. Postoperative Concerns

This chapter will discuss routine care and monitoring of pediatric patients postoperatively. It will include controversies about discharge criteria, postoperative pain management, and common complications such as nausea/vomiting and stridor.

VI. Specialty Anesthesia

This chapter will include specifics about managing common pediatric surgeries. It will be organized into subspecialty areas that will include: ENT, Cardiac, Ophtho, Ortho, Thoracic, Abdominal, Neonatal emergencies, Dentistry, Urology, Neurosurgery, Transplants, Radiological procedures, Radiation therapy, Endoscopy, and Sedation for painful procedures.

VII. Critical Care

This chapter will introduce the resident to the essentials of pediatric critical care. Since the ABA does not require too much knowledge in this area, this chapter will not be very detailed, but will include principles of resuscitation (PALS & NALS).

VIII. Acute & Chronic Pain Management

This chapter will briefly review principles of acute and chronic pain management in children.

Case Studies

At the end of each chapter there will be one to three case studies that are related to the chapter content. These can be contained within the gray boxes, or the gray boxes can be reserved for more traditional lists, or tables. Each case study will consist of a case and several questions to be answered about the case. The answers can either be listed directly below the questions, or preferably, further down so the reader can formulate their own answer first. The case study will ideally describe a controversial issue.

Example of case study from the Postoperative Concerns chapter:

A 3-month-old child presents for inguinal hernia repair. The child was born at 25 weeks gestation, and was ventilated for the first three weeks of life. Ongoing medical problems include anemia, and chronic lung disease of infancy.

Question 1. What are the common medical problems in former premature infants?

Question 2. What is the safe level of hemoglobin prior to anesthetizing this infant?

Question 3. What's the safest method of anesthesia for this infant?

Question 4. What criteria should be used to safely discharge this infant home following surgery?

Lee Fleisher, a national leader in anesthesiology, has continued the tradition of previous editions with this fine book.The comprehensive chapters stand by themselves as a thorough reference.Conversely, the summary tables and strategies for diagnosis and management give this edition great range for 'middle-of-the-night' suprises as well.

The book is made coherent by consistent chapter design.Authoratative and well-written, it belongs in every anesthesiologist's library.

I have used each edition of this book as a reference, but was so disappointed with the 5th edition that I returned it after 1 week. This was once the "go to" book for the rare and esoteric. No more. For example, there is nothing "uncommon" about the problems addressed in the chapters on trauma and acute care, herbal medications, or pregnancy. The chapter on congenital heart disease (CHD) focuses on pediatric patients, a common subject with books of its own. A chapter on the adult patient with palliated or corrected CHD would have been more useful.
The chapter on muscle diseases is very disappointing compared to the excellent one by Miller and Rosenbaum in the 4th edition. The chapter on hematologic disorders lumps impaired coagulation and prothrombotic disorders with diseases of platelets. A discussion of acute blood loss is duplicated here and in the chapter on trauma.
There are 2 significant omissions: no chapter on cancer patients or chemotherapeutic agents (hardly uncommon but outside the realm of most anesthesiologists), and no chapter on rheumatic diseases like lupus or rheumatoid arthritis that affect multiple systems and have novel therapies.
The 5th edition is an unwelcome departure from the original concept of Katz and Kadis. Perhaps the 6th edition will capture the spirit of what was formerly an awesome reference text. ... Read more

While Mr. Cottrell provides some helpful information to patients with concerns regarding anesthesia, his unbiased negative perspective of nurse anesthetists is a disservice to the public. It is "his" opinion that obstetrical anesthesia should be done only by physican trained anesthesia providers instead of nurse trained anesthesia providers. Perhaps he should review the recent study, Anesthesia Staffing and Anesthetic Complications During Ceserean Delivery: A Retrospective Analysis. Nursing Research, 2007;56(1): p. 9-17. The study found no difference in complications of the administration of anesthesia in labor and delivery in hospitals that employed only CRNAs (Certified Registered Nurse Anesthetists) vs. hospitals that employed only anesthesiologists. In fact the CRNA-only hospitals see greater percentage (43% vs 28%) of Medicaid patients than anesthesiologists-only hospitals. Nurse Anesthetists provide 60% of anesthesia and 90% of anesthesia in rural areas. CRNAS --Lets not bash this important and safe provider of anesthesia.

I found this book to be a very good source of information and advice, written in a style that was easy to follow and understand. My only disappointment was that it wasn't longer. Being disabled with painful back problems, I really appreciated the inclusion of a chapter on chronic pain management.
