A general practitioner with a list size of 2000 patients will see about 20 patients with heart failure each year and 10 of whom will be new cases . The general practitioner can expect four admissions to hospital in one year with patients with heart failure . Half of the patients diagnosed with heart failure will die within four years and half of those with severe heart failure will die within one year . It has been estimated that 1% of the NHS budget is devoted to heart failure management, with 60% of this spent on hospital admission

Extractions: slusignan@drs.desk.sthames.nhs.uk The Heart Failure at home project involved teleconsulting and measuring patients vital signs at home (Pulse, BP and Weight.) The vital signs data is stored on a Telemedicine server located at St George's in London - this is located within NHSnet (the NHS own secure Intranet.) It can be accessed by the specialists (in our case in Guildford and Oxford) as well as by the practice. It is also accessible by the GP and community nurse. Most of the patient care has been provided by Community Nurses, and this set-up has given them direct links to the Consultants involved in patient care in a way they would not have usually had. Our pilot project is due to end 31st March 1999, and we are looking for further collaborative funded research from after this date. The department provides adult and paediatric echocardiograms, adult transoesophageal echocardiograms and reports. Exceptional cardiac and cardiothoracic care, as well as treatment of advanced heart failure, is provided by UW physicians at Harborview and UW Medical Center Under the umbrella of the University of Washington Regional Heart Center , the Cardiology Clinic at Harborview provides echocardiography laboratory and management services for adult patients with cardiovascular diseases. Harborview also has the ability to perform diagnostic heart catheterizations and acute interventions. Adult patients whose primary diagnosis is mild to moderate cardiac dysfunction and have an expected length of stay less than 72 hours are admitted to Harborview's Acute Cardiac Evaluation Unit. Also known in the community is Seattle's Medic One that was developed by Harborview's chief of cardiology in the 1960s with the Seattle Fire Department. This was one the first programs of its kind in the world and is the program most often emulated by other communities. The program trains paramedics for the Seattle Fire Department and for King County, as well as for other services in the state. Diagnostic catheterization allows us take pictures of the heart and vessels, to measure pressures, and enables us to obtain important information on the heart condition in order to determine if an operation is needed.
