All are infamous for the rash of school shootings of the late 1990s, each unwillingly serving as poster children for every cause from gun control to restoration of prayer in school. Tollerud helped to launch the Illinois School Counselors Academy (ISCA) last fall as a collaborative effort between the Regional Offices of Education, the Intermediate Service Centers, the Chicago Public Schools, the Illinois School Counselors Association, the Illinois State Board of Education and Education-to-Careers. Cathy Shelton, a counselor at Clinton High School in Clinton, Ill., first proposed the organization. Tollerud already has spoken with more than 700 educators across the state. She is referring to major depressive disorder (MDD), a recurrent, incapacitating illness characterized by debilitating changes in sleep patterns, appetite, mood, and memory lasting at least two weeks. Hauenstein was struck by the prevalence of MDD among rural women of childbearing age while working with chronically ill children at U.Va. during the 1980s. "Sick kids are a serious drain on maternal resources, and a great many of the moms were depressed," she recalls. Poverty and rural life in all its manifestationslack of health insurance, transportation, and child careconspired to prevent these women from taking advantage of the mental health services they needed. Hauenstein noted that when these women sought health care at all, they tended to use primary care settings. Accordingly, when she applied for a research demonstration grant from the National Institute of Mental Health, she made the rural community health centerand not the large regional hospitalthe point of entry for women needing treatment for MDD.

This project consists of two stages: the first stage involved qualitative studies on ageing and mental health in Goa. Nine focus group discussions were conducted with elders, carers and health workers. The findings showed that although families were the main carers, many elders lived alone and there were many instances of neglect and abuse. These findings have recently been published in Psychological Medicine. The second step aimed to develop culture and education fair measures of diagnosis of dementia and depression in the aged in community settings in the Konkani language and to obtain information on care arrangements and carer burden in families with elders with and without dementia and depression. This stage was conducted in the communities of Mandur and Santa Cruz. The study involved recruitment of 30 subjects with dementia, 30 with depression and 60 "normal" elders. The findings have shown clearly the enormous economic and health impact of caring for dementia. Most carers are women, particularly spouses, and caring for dementia leads many to become depressed.
