    The Crag Rats are the oldest volunteer rescue organization in the United States. They operate in the Cascades, responding whenever they're able to the call for help in finding lost hikers - getting injured waterfall jumpers into the hands of emergency transport - even recovering bodies after those they might have rescued are past the need for that. They do this at more personal cost than providing their own equipment. For emergency room doctor Chris van Tilburg, his volunteer rescue activities create marital tension and take away family time even though he sets careful limits on that loss. Yet this is how he has always wanted to live his life, and he makes a tremendous difference for those who need him.

    The chapters of this book take the reader through rescues in all four seasons, some of them hearteningly successful and others heartbreakingly futile. The author describes nature honestly and with love even when nature is being anything else but kind. He is just as frank about humans and their interactions. This makes for an exciting read most of the time, although some chapters do bog down into excessive detail. My only other quibble is St. Martin's surprisingly inept copy editing. I don't expect any publisher to catch all of a book's typos and other mistakes these days, but this one's numerous errors pulled me out of the story far too often.

    "Mountain Rescue Doctor" is a memoir describing what it's like to be a part of a mountain "search and rescue" team. The author told many vivid, suspenseful stories about a variety of different rescues--some easy, others hard--over a particularly active year. The author is a doctor and often had to do emergency medical care under some extreme circumstances, but it was easy to follow what he was doing and the descriptions of the injuries weren't gory.

    He also included information about the dangers of mountain hiking, biking, and climbing; a brief history of rescue teams in general and his group in particular; how the different types of searches are handled; the mundane aspects of being a SAR (Search and Rescue) member--like meetings and training; how being a on-call SAR member effects his family; why he picked this job and how he got training for it; and some stories about his recreational mountain climbing trips.

    There were also several pages of impressive, full-color photos from several of the rescues that he described. Overall, I found this memoir well-written, very interesting, and hard to put down. I'd recommend it to anyone who likes extreme sports or who thinks the topic sounds interesting.

    Imagine sticking a breathing tube down someone's throat. And imagine having to do that without accidentally inserting the tube into the person's esophogus or breaking the patient's teeth. Then imagine doing this while kneeling on sharp rocks while perched on a narrow ledge, as a hovering rescue helicopter sprays you and your patient with dirt and debris. That's what Dr. Christopher Van Tillburg had to do during one of the many wilderness Search & Rescue missions he's been on as a member of the Crag Rats, the oldest SAR team in the U.S.

    As a Search & Rescue volunteer myself, I'm hooked on true stories about wilderness rescue and survival, written by both rescuers and those they help, or by those who made it out of difficult situations on their own. Having read many, this one is still near the top of my list of favorites. My only disappointment when I finished the book was that, in certain cases, I was left wondering what happened to some of the victims Dr. Tillman had worked so hard to save. Did they survive? Then I happened across a blog post by the author, in which he states, "Yes, several chapters don't really say what happens to the patient. That's part of the deal with mountain rescue missions: we hand off the patients to a helicopter or ground ambulance crew and sometimes we never find out the end result." As any member of a SAR unit knows, that statement is very true. The last we sometimes see or hear of a patient is when they're whisked into the sky in a litter, spinning at the end of a 200-foot rope.

    Mountain Rescue Doctor



    I just finished reading the book "Mountain Rescue Doctor" by Christopher Van Tilburg and I have to say it was a great read. I literally could not put it down. He gives multiple perspectives of mountain rescue and addresses many of the debatable topics when it comes to who should bear the financial costs of rescue, etc.

    This book is great for both the outdoor enthusiast or anyone who just enjoys getting up to the mountain every now and then for some leisurely riding. There are great lessons to be learned from Van Tilburg's accounts and his writing makes you respect both the mountain and those that serve to aid in the rescue of the lost, injured and deceased

    Personally is the first one that I have read with this subject and has been very useful in my practice as a General Surgeon dealing with patients and teaching medicine students and surgery residents.

    I'm an EM resident studying for In-Service exams as well as preparing for the Board test in a couple of years. I like the explanations immediately following questions so I don't have to leaf through the book constantly. The quality of the questions is pretty good, and very comparable to the 1,000 question book.

    I'm a practising EP and just successfully completed my 1st ABEM recert.

    While I did pick up a few excellent pearls reviewing with EM Q&A, overall, I found it wordy, confusing and low yield.A small but substantial number of questions were not even relevant to EM... I mean, a question concerning biopsies!Come on guys, what happened to Pearls?

    But unless you are a Doctor or want to go to prison, don't practice most of the proceedures in this book.
    I loved it anyway even though I am a humble medic.

    "Ditch Medicine" is very informative and is written in a concise manner.
    No single publication contains all the information that we would like but this book should be in your personal library if you have an interest in emergency field medicine in an austere environment. I highly recommend this book.I also highly recommend emergency medical training prior to employing many of the procedures outlined, if at all possible.

    Not just how to treat an amputation, but surgical procedures for performing an amputation!Other topics include IV cutdowns, nerve blocks, chest tubes, and suturing.While the procedures are numerous, I can't imagine learning how to perform them successfully from the text alone.The author assumes his reader is well-equipped with both gear and a basic knowledge of medicine.If you have the proper gear and can understand the instruction, you probably didn't need the book.Further, if you have no experience with IVs or suturing, performing a cutdown seems ambitious.As stated by another reviewer, much of the information feels dated.Even moreso, extreme amounts of information are left out.Nevertheless, while the topics in this book are covered better elsewhere, it is still interesting.Useful?Probably not.I can't say the instruction is terribly practical.Neither do I find it helpful to reference.But it definitely has cool factor and I'm glad I have it!

    For the cost this is a good reference.It is written in plain english and is easy to understand.It is of particular use to the health care professional that has never operated or has limited experience in austere environments.It has some deficiencies (pictures/drawings are not the greatest, some of the material is dated, no real discussion of permissive hypotension etc...) but these are forgivable in a volume of this size, cost and ease of reading.

    Excellent resource.I have already used it in my practice.I work in the ER so it helps to ensure that I am performing the right techniques and making proper referrrals.

    I am a PA in an ED and this book is invaluable, although I did an ortho rotation this book gives the current thoughts/treatment modalities for injuries. I have found that it deliniates what needs surgery now vs. in the near future or what can be splinted and follow up. (That is always the tough thing to figure out without experience.) This is an easy read and great from the beginning up to advanced ortho. It is well organized and goes by sections of the extremities (upper arm vs lower arm, hands then fingers). It has helpful sections on MRI, CT and other basic radiology studies for ortho purposes. Well worth the money!

    in the years past, rosen's used to serve as "the" reference textbook in EM. recently, however, other textbooks (e.g., harwood-nuss) bave become very comparable in breath and depth, for less money. btw, one can use the electronic version without the cd's! ("drchief, DO" was probably skipping his english class and pc 101 in his osteopathic curriculum)

    While the content of the textbook is excellent, the mechanism of use is horrible.Previous versions allowed you to load the textbook into your laptop for ease of use.With this CD, it must be in the CD drive for you to view the book, as it is browser based.I am not exactly why they made this change, but I would not have purchased the item had I known.It has proven to be more trouble than it is worth, having to load the CD for each use of the book.I use the prior edition due to it's ease of use. ...     I can highly recommend Marini/Wheeler to all Pulmonary/Critical Care Medicine fellows and all those practising Critical Care Medicine physicians who want to brush up their memories when something out of the ordinary occurs during rounds. It is easily transportable, not like some of those 20 pounds textbooks one finds in the libraries (and where they should remain!). I used Marini/Wheeler as a base for my Critical Care Medicine Boards in 1995 and subsequent recertification and only occasionally opened one of these huge textbooks for more detailed information my then chief had recommended at the begin of my Critical Care Medicine fellowship (which, dutifully, I had bought, only to let them collect dust on my bookshelves afterwards).

    Not bad, but the writing style is somewhat dense, making it difficult to read quickly. After a couple chapters I decided to switch to the ICU book by Marino and was happy I did.If you're a busy resident with limited time to read, go with Marino.

    An excellent discussion of ICU/Critical Care Medicine pathophysiology and management approaches for principally the housestaff/junior fellow level.Chapters are pertinent and understandable.Does a good job of avoiding the tendency toward "cookbook" medicine found in several other CCM texts written at this level.End of chapter summaries are excellent.Focus is on developing understanding of key physiologic/pathophysiologic concepts and how they relate to monitoring, management, etc.rather than focusing on procedures and techniques.Should be the text of choice for medicine critical care rotations early in training as it really develops the appropriate foundation. ...     Hi, I am one of the medicine residents. I met this book in my intern year. This book is extremely compact and easy to go through. Even in middle of the ER rotation, we can check "what kind of order I should get??", "What's the treatment??", "What's the key history and PEx??". Even though I kinda used to work in ER already, I try to carry this book since I am not working in ER all the time. This is the "best reminder" for me when I see patients who has any problems outside of internal medicine (I think I know internal medicine...)

    I found this pocket sized book indispensible for ER rotations since it gives you a USEFUL concise one page review of commonly encountered emergencies. Each diagnosis goes through etiology, presentation, physical exam, differential, recommended imaging/labs and disposition options. Use this as a memory aid if you are a resident and as a med student to look like a star! Granted Tintanelli is way more expansive and detailed, but not as portable. I do NOT recommend this as your sole option during a rotation for learning the theory and details behind everything because it will tell you what to do without alot of why. ALSO, KEEP IN MIND the latest edition is from 2002! The basics don't change much, but I would still love to see a new edition! ... Read more

    great book, using it to review for the boards. the pics of pediatric rashes are particularly helpful. it is a bit lacking on the radiology side of things, and i wish this book had a section on spinal trauma, especially c-spine injuries. (hint, hint, for the next edition) otherwise a great reference to have.

    Let's first say that this book is not for the faint of heart. But if you're a medical professional and efeects of trauma and injuries on the human body intrest you , this is an excellent book. Full color graphic pictures of numerous typrs of injuries caused by trauma and various other causes showcase just how frail the human body can be. Seeing injuries in the book prepare you to better recognize them faster in the field and prepare you for the shock of seeing them for the first time in the field. Treating injuries in the field can better be accomplished through fast recognition of a patients treatment needs if you've have never dealt with a type of trauma/injury before and having seen it in the book can help you familiarize yourself beforehand. 4 Stars! ...     There is probably no other book of this size that has such an impressive scope of procedures available to the pediatric resident, emergency medicine resident, or ICU fellow available. The number of procedures, and the accompanying diagrams are truly amazing!

    A "quick reference" handbook should be small enough to tuck in a pocket -- and this one is.It should be comprehensive enough to cover the more common problems for which quick information is needed -- it does!Andit should be organized in a fashion that makes retrieval of informationquick and easy -- again it succeeds.There's just too much data involvedin the practice of Emergency Medicine to remember it all with completeaccuracy, and this little "peripheral brain" gives accurate,relevant and critical data when the emergency physician most needs it. It's not just for the new EM physician, but is a gold mine of info for the"old gray-hair" with years of experience!The only fault I findis that there is need for a new edition -- new drugs, new ACLSrecommendations, new technology have come on the scene since this 1995edition was first put in print.There's so much useful data that it stillshould be at every EM physician's workstation -- or better yet, in thepocket of every EM doc's lab coat! ...
