Of these, one year is an elective. The program has received national recognition, being ranked among the best programs in the country by the United Network of Organ Sharing and US News and World Report. During the 10 years that it has been in existence,the average resident has published more than six papers and has received clinical training that allows him or her to compete successfully in both the academic or private practice setting. The PGY-3 year is spent entirely at the University of Massachusetts Medical Center, rotating on either the adult or the pediatric/transplant service for a period of six months each. The PGY-4 through 6 years give the resident a considerable amount of autonomy: in each of these three years, the resident functions as a senior resident on the particular service that he or she IS rotating. The PGY-4 is split between the Medical Center of Central Massachusetts and the Togus VA Medical Center. The PGY-5 year is an elective year, where a range of electives offered by the Division can be explored. One popular rotation has been at the Hospital for Sick Children, Great Ormond Street, London, England, under the tutelage of Mr. Philip Ransley. A second option has been in the Urological Research Laboratories at the University of Massachusetts Medical School Campus. This determined to a large extent by the resident's interest and long term goals. The PGY 6 year is spent as the Chief Resident on the Urological services of the University of Massachusetts Medical School Campus and the St. Vincent Hospital. In addition to a strong research and academic experience, the residents average dround 500 to 600 operative procedures a year encompassing the entire spectrum of urology and renal transplantion.

What's New in Pediatric Urology, 2001 Infections The presence of even a single episode of epididymitis in children has been considered an indication for evaluation to detect a renal or ureteral anomaly as its etiology. A study from Paris challenges this assumption with data from evaluation of 38 children seen over an 8-year period (Cappele et al., 2000.) They found that only 18% of evaluated children had renal anomalies such as reflux or renal malrotation, and only one child required surgery (for ureterocele.) The frequency of anomalies was lower than expected. Bladder anomalies Reflux A survey of American Association of Pediatrics, Section on Urology members on standard management of vesicoureteral reflux (Herndon et al., 2001.) Urine culture is routinely performed by 64-71% of respondents and yearly VCUG or radionuclide scan for followup by 99% of respondents with ultrasound by 77%. After reflux surgery, 91% of respondents perform VCUG and ultrasound. The overwhelming majority of practitioners agree on the timing and type of radiographic studies to be used to follow children with reflux. A low incidence (2%) of renal scars in children after ureteral reimplantation was noted in a prospective study from Australia (Webster et al., 2000.) These findings suggest that surgical correction of reflux may protect kidneys better than previously believed.

Testicular cancer occurs in approximately 1 in 25,000 men per year. It is 4 times less common in Afro-American men compared to Caucasian men. The risk of developing testicular cancer in a mans lifetime is approximately 1 in 500. It occurs most commonly between the ages of 15 and 40 years. It can also occur in infancy and late adulthood (e.g. over 60 years old). Individuals who have had an undescended testicle are at higher risk of developing testicular cancer. How is testicular cancer diagnosed? The best way to diagnose testicular cancer is through self-examinations . The usual initial finding is a painless lump in or on the testis, a hardness or enlargement of the testis. Less commonly there is associated pain and tenderness. If problems do occur and an individual is concerned due to tenderness, bloody discharge or a lump for some symptoms they should see a urologist. The urologist will do a physical examination and possibly order a testicular ultrasound. Non-cancerous masses found around the testicle can be fluid collections or cysts.
