NASA announces Columbia accident investigation board

NASA Administrator Sean O'Keefe today announced the members of the Space Shuttle Mishap Interagency Investigation Board, which will provide an independent review of the events and activities that led up to the tragic loss of the seven astronauts Saturday on board the Space Shuttle Columbia. The board's first meeting is scheduled for tomorrow at Barksdale Air Force Base in Louisiana. Retired U.S. Navy Admiral Harold W. Gehman, Jr., who co- chaired the independent commission that investigated the attack on the U.S.S. Cole in Aden, Yemen, Oct. 12, 2000, and once served as the commander-in-chief of U.S. Joint Forces Command, will chair the panel. "While the NASA family and the entire world mourn the loss of our colleagues, we have a responsibility to quickly move forward with an external assessment to determine exactly what happened and why," said Administrator O'Keefe. "We're honored to have such a distinguished panel of experts, led by Admiral Gehman."

Short Courses Aircraft Accident Investigation 19 May -27 June 2003 Cranfield University, UK Introduction
This six week course, the twenty-six to be devised by the Human Factors and Air Transport Department in cooperation with the Air Accidents Investigation Branch of the Department of Transport, is based on the AAIB's considerable international experience and has been developed from the previous successful courses.
The aim is to bring together people from many areas of aviation activity to evaluate how accidents happen and provide a sound basic knowledge of all the requirements, procedures and techniques associated with accident investigation and prevention.
Who should attend
The course is primarily designed for personnel from all parts of the civil aviation industry, however some military personnel are also accepted. It will be of particular value to those who may have to deal directly or indirectly with an actual accident investigation whether as an official investigator for a participating State or as a representative of a manufacturer or an airline concerned.
Members of the course normally include pilots, engineers and air traffic controllers, all of whom are introduced to the skills of their colleagues working in the field of aircraft accidents investigation.

The Cranfield Aviation Safety Centre together with the Air Accidents Investigation Branch, Department of Transport has run a two-month course on Aircraft Accident Investigation each year since 1977. Following discussion with the British Airline Pilots Association we devised this two-week course to specifically reflect the needs of aircrew who may be involved in the operational and human factor aspects of an accident. The aim of the course is to bring together a group of experienced people for discussion and evaluation of how accidents happen and in particular to provide them with a basic knowledge of accident investigation techniques and international procedures.
Course notes will be provided and a copy of the ICAO Manual of Aircraft Accident Investigation will be available for reference during the course.

Only after many months of official investigation is a final accident investigation report published. That report may contain hundreds of pages of highly technical information. Generally, each issue of Accident Prevention Accident Prevention fills the gap between nonspecialist reporting, which can be superficial (and inaccurate), and official accident investigation reports, which typically are lengthy, highly technical and not readily available to the public. About ICAO

The International Civil Aviation Organisation (ICAO), the specialised agency of the United Nations (UN) for civil aviation, was created by the Convention on International Civil Aviation signed in Chicago on 7 December 1944. Its aims and objectives are to develop the principles and techniques of international air travel navigation and to foster the planning development of international air transport. It also sets international standards and regulations necessary for safe, regular, efficient and economical air transport, and serves as the medium for co-operation in all fields of civil aviation among its 187 Contracting States.
Another of ICAO's functions is to facilitate the adoption of international air law instruments and to promote their general acceptance. ICAO also conducts workshops in various regions to provide States with information and advice on its activities and to facilitate exchange of information and views. In addition, ICAO also provides assistance to States to improve their aviation security facilities and procedures.
In recent years, ICAO has undertaken extensive work in areas like reporting aircraft accident and incident data, and automation of air traffic services. Among ICAO's more significant achievements has been the development of a satellite-based concept to meet the future communications, navigation, surveillance and air traffic management needs of civil aviation.
