There are very few developing countries in the world where public policy has been effective in preventing the spread of HIV/AIDS on a national scale, but Thailand is an exception. A massive program to control HIV has reduced visits to commercial sex workers by half, raised condom usage, decreased STDs (Sexually Transmitted Diseases) dramatically, and achieved substantial reductions in new HIV infections. However, unless past efforts are sustained and new sources of infection are addressed, the striking achievements made in controlling the epidemic could now be put at risk AIDS had occurred in Thailand by 1984, but the initial policy response was limited. The prevailing view was that this was an epidemic brought from abroad that would be confined to a few individuals in high-risk groups, like gay men and injecting drug-users, and would not spread more widely. In 1988-89 that view was challenged In the first major wave of the epidemic, HIV infection exploded among injecting drug-users, rising from almost nil to 40% in a single year. At almost the same time a second wave of infection spread among sex workers. In 1989, it was found that 44% of sex workers in Chiang Mai, in the north, were infected with HIV. The rising infection level among sex workers, which reached 31% nationally by 1994, launched subsequent waves of the epidemic in the male clients of sex workers, their wives, and partners, and their children. In 1990-91, the Government acted decisively, launching a nation-wide campaign to reduce HIV transmission. The key elements of the program were a massive public information campaign launched through the media, government, and NGOs and a program to promote universal and consistent condom use in commercial sex. The response was lead by a multi-sectoral National AIDS Prevention and Control Committee, chaired by the Prime Minister. It actively engaged NGOs and the population in general and the results were dramatic. Fewer men went to brothels, condom use in brothels rose to more than 90%, the number of consultations at STD clinics was reduced by 90% and the infection rates among army conscripts dropped by half in only a few years.

HIV/AIDS in Namibia HIV/AIDS Programme The most important and far-reaching developmental crisis facing Namibia today is the HIV/AIDS pandemic. Due to the political and social isolation experienced by Namibia and South Africa during the 1980s, the pandemic started later here than it did in neighbouring Zambia, Zimbabwe or Botswana. However, after nearly 20 years, the HIV/AIDS crisis in Namibia, as in the Southern African region generally, continues to rage. Namibia currently ranks as the seventh most HIV/AIDS infected country in the world with an overall prevalence of just under 20% among the adult population. One in five Namibians aged 15-49 are infected by HIV, implying that the demographic structure of the population will change significantly, resulting in a small number of young adults having to support a large number of young and old people. By the end of 1999, the estimated HIV prevalence rates for the 15-24 year age group ranged from 19 percent to 21 percent for females as compared with a corresponding range of 8 to 10 percent for their male counterparts. The high level of HIV rate in Namibia and its impact on the life expectancy, alongside the already mentioned high level of inequalities, can also justify the relative low HDI level of Namibia in 2000 despite a positive growth of the economy.

Available is the newest (February 2002) version 11 of the electronic Textbook of AIDS Pathology which is approximately 800k (including text, tables, graphics, and references). The textbook includes a general discussion of the pathophysiology of HIV infection, organ system pathology of AIDS, and descriptions of the opportunistic infections and neoplasms associated with AIDS, and a general discussion of issues, including safety and education, related to the AIDS epidemic.

Human Immunodeficiency Virus
A tutorial about HIV and AIDS is available. Go to the HIV tutorial. Human immunodeficiency virus (HIV) is the causative agent for AIDS. The most common type is known as HIV-1 and is the infectious agent that has led to the worldwide AIDS epidemic. There is also an HIV-2 that is much less common and less virulent, but eventually produces clinical findings similar to HIV-1. The HIV-1 type itself has a number of subtypes (A through H and O) which have differing geographic distributions but all produce AIDS similarly. HIV is a retrovirus that contains only RNA. HIV is a sexually transmitted disease. Infection is aided by Langerhans cells in mucosal epithelial surfaces which can become infected. Infection is also aided by the presence of other sexually transmitted diseases that can produce mucosal ulceration and inflammation. The CD4+ T-lymphocytes have surface receptors to which HIV can attach to promote entry into the cell. The infection extends to lymphoid tissues which contain follicular dendritic cells that can become infected and provide a reservoir for continuing infection of CD4+ T-lymphocytes. HIV can also be spread via blood or blood products, most commonly with shared contaminated needles used by persons engaging in intravenous drug use. Mothers who are HIV infected can pass the virus on to their fetuses in utero or to infants via breast milk.
