Sleep Apnea My husband, Norman, often fell asleep watching television, and the kids snickered. "Dad's getting old," my daughter would whisper. But Dad wasn't really old. In his early fifties, he was much too young to be sleeping his life away. As the malady became worse, he'd fall asleep a dozen times a day, even in the middle of a sentence. He often fell asleep while driving, creating a danger to himself and to others. If I were there, I'd nudge him with my elbow or shout to wake him up. Several times, however, he was alone and he landed in the ditch. He loved his fishing, but eventually he had to give the sport up because of the danger of going to asleep and falling out of the boat.

Sleep apnea afflicts more than 12 million people in the United States. It takes its name from the Greek word apnea, which means "without breath." People with sleep apnea stop breathing repeatedly during their sleep, often for a minute or longer, and as many as hundreds of times during a single night. Sleep apnea can be caused by complete obstruction of the airway (obstructive apnea) or partial obstruction (obstructive hypopnea; hypopnea is slow, shallow breathing), both of which can disrupt sleep. There are three types of sleep apnea — obstructive, central, and mixed. Of these, obstructive sleep apnea (OSA) is the most common. OSA occurs in approximately 2% of women and 4% of men over the age of 35. Physiology and Causes The cause of OSA is unclear. The site of obstruction in most patients is the soft palate, extending to the region at the base of the tongue. The body has no rigid structure, such as cartilage or bone, in this area to keep the airway open. During the day, muscles in the region keep the passage open. As a person with OSA falls asleep, these muscles relax to a point where the airway collapses and becomes obstructed. When the airway closes, breathing stops, and the sleeper awakens to open the airway. The arousal from sleep usually lasts only a few seconds, but brief arousals disrupt continuous sleep and prevent the person from reaching the deep stage of rapid eye movement (REM) sleep, which the body needs to rest and replenish its strength. Once normal breathing is restored, the person falls back to sleep, only to repeat the cycle throughout the night.

Why do we snore?
The sound of snoring occurs when air cannot flow freely in and out of your throat. This is due to a narrowing of the passages at the back of the mouth and nose. During sleep, muscle tone decreases throughout the body. When your throat muscles relax, the soft palate and uvula (the fleshy structure that dangles from the roof of the mouth) vibrates as you breathe in and out. Snoring tends to become worse with alcohol or sedative consumption before bedtime, weight gain and a blocked nose. Snoring often increases with age. The most common kind of sleep apnea is called Obstructive Sleep Apnea Syndrome. Sleep apnea means "cessation of breath." It is characterized by repetitive episodes of upper airway obstruction that occur during sleep, usually associated with a reduction in blood oxygen saturation. In other words, the airway becomes obstructed at several possible sites. The upper airway can be obstructed by excess tissue in the airway, large tonsils, a large tongue and usually includes the airway muscles relaxing and collapsing when asleep. Another site of obstruction can be the nasal passages. Sometimes the structure of the jaw and airway can be a factor in sleep apnea. What are the symptoms?
  • excessive daytime sleepiness
  • frequent episodes of obstructed breathing during sleep. (The patient may be unaware of this symptom usually the bed partner is extremely aware of this).
Associated features may include:
  • loud snoring
  • morning headaches
  • unrefreshing sleep
  • a dry mouth upon awakening
  • chest retraction during sleep in young children (chest pulls in)

What is Infant Sleep Apnea? Infant sleep apnea is characterized by central apneas or obstructive apneas that occur while asleep. An apnea is the stoppage of airflow at the nostrils and mouth lasting at least 10 seconds. A central apnea is when respiratory efforts stop for more than 10 seconds (20 seconds in infancy). An obstructive apnea is when the upper airway is blocked. Up to what age is it considered "Infant" Apnea? The diagnosis of 'Apnea of Infancy' is reserved for infants who are older than 37 weeks (approximately 9 months) at the onset of the apnea and for whom no specific cause of apparent life-threatening event (ALTE) or apnea can be identified. 'Apnea of Prematurity' is restricted to apnea in infants younger than 37 weeks (approximately 9 months) and not due to an explainable cause except respiratory immaturity. What are the symptoms?

Chronic Obstructive Airways Disease
There can be large falls in arterial oxygen saturation during sleep in patients with chronic obstructive pulmonary disease. In general, these falls can be predicted from daytime oxygen levels and are not due to classical OSA syndrome, but to a number of factors which combine to produce worsening hypoxaemia during sleep. These patients are often at the limits of their respiratory capacity, and the loss of the intercostal and accessory muscle contribution to breathing, and the increase in upper airways resistance associated with REM sleep, cause marked falls in ventilation. In addition, patients may be hypoxic while awake and small falls in ventilation and PaO2 lead to large falls in arterial oxygen saturation during sleep. The hypoxic episodes demonstrated by overnight oximetry in a patient with chronic obstructive pulmonary disease differ in character from those seen in patients with OSA syndrome (figure 10). Figure 10. Overnight arterial oxygen saturation monitoring in a patient with chronic obstructive pulmonary disease. There is some evidence that increases in upper airway narrowing during sleep, short of apnoeas and even snoring, may contribute to the development of CO2 retention in these patients. However classical sleep apnoea is not significantly more common in unselected COPD patients with raised CO2 levels, and excessive nocturnal hypoxaemia is not one of the predictors of long- term survival in patients with COPD.

Optovent has developed a combination instrument, Respons , which is a new portable instrument for real-time respiratory monitoring in combination with pulse oximetry. The instrument monitors and records a patient's breathing pattern and oxygen saturation , and issues alarms upon detection of user-defined abnormalities such as apnea and hypoxemia.
