Bipolar Disorder (Manic Depression)
Overview of Depression Depression is a depressive disorder that involves a child's body, mood, and thoughts. It can affect and disrupt eating, sleeping, or thinking patterns, and is not the same as being unhappy or in a "blue" mood, nor is it a sign of personal weakness or a condition that can be willed or wished away. Children with a depressive illness cannot merely "pull themselves together" and get better. Treatment is often necessary and many times crucial to recovery. Three types of depression: There are three primary types of depression, including:
  • major depression (clinical depression) bipolar disorder (manic depression) dysthymia
    (dysthymic disorder)
What is Bipolar Disorder?
Bipolar disorder, also known as manic depression, is classified as a type of affective disorder (also called mood disorder) that goes beyond ordinary ups and downs, becoming a serious medical condition and important health concern in this country. Bipolar disorder is characterized by periodic episodes of extreme elation, happiness, elevated mood, or irritability (also called mania) countered by periodic, classic major depressive symptoms.

Research into manic depression Does manic depression run in the family?
As with depression and schizophrenia , there is also a genetic element to manic depression. In fact, there is a very strong genetic influence, for example, if you are an identical twin and have manic depression then there is a 75% chance that your twin will also suffer from the illness. If your mother or father or brother or sister has manic depression then there is a 10% chance of you becoming ill. If a more distant relative has the illness then there is little, if any, increased risk that you will suffer from it.
Manic depression and diet
Food allergies may cause severe mood swings in some people. This could provide some support into the theory that manic depression may be helped by changes in diet. If you suffer from manic depression, a well-balanced, healthy diet could help your body to fight off mood swings more effectively. Some people claim that deficiencies in Vitamin B complex, B1 (Thiamin), B6, B12, folic acid, magnesium, calcium, manganese and zinc have all been linked to mood disorders, and that taking food supplements and vitamin pills can help.

Manic depression Most of us depression and one episode of mania. If you experience mania:
  • you may feel very 'high' and become very active, rushing around
  • your sleeping patterns may change, you may go to bed very late and get up very early
  • you may talk a great deal and move quickly from one subject to another
  • you may also feel all-powerful and capable of achieving anything.
Unlike schizophrenia, where 80% of people receive a diagnosis between the ages of 16 to 25, people with manic depression can be diagnosed from puberty to old age, although a large number of people are diagnosed for the first time in their thirties. If you are a woman you are much more likely to have manic depression than if you are a man. FACTS ABOUT MANIC DEPRESSION
from the flyer of the same name sponsored by the Missouri Council for Comprehensive Psychiatric Services Roller coaster rides of emotion from frantic highs to devastating lows are the classic signs of manic-depressive illness. Also known as bipolar disorder, manic depression is one of the most treatable mental illnesses, but left untreated it can cause mental suffering, disruption of family life, poor job performance, and reckless or dangerous behavior.
Symptoms
A person with manic-depressive illness experiences mood swings from mania to depression, with a "normal" period between these cycles of up and down. The length of the cycles varies from a few days to several months and they can occur without warning. During the manic phase, a person may feel "on top of the world" and have an abundance of energy. He seems to talk and think faster and espouse a number of ideas. He also may think he is invincible, leading to reckless behavior and acts that may endanger his life or well-being. A person in the manic phase also may have delusions of fame or believe she has a special relationship with a famous person. During this phase she also sleeps less, is easily distracted, and often is irritable. During the depressive phase, a person may feel hopeless and lose all interest in other people or usual activities. Her weight may fluctuate and she feels tired all the time. She may sleep more than usual or have insomnia. She often complains of unexplained aches or pains and has trouble concentrating. A person in the depressive phase is a suicide risk.

One in 150 people suffer from manic depression
Manic depression is genetic and is caused by an excess of signalling cells in the brain, say scientists. Research carried out in the US has found that people with manic depression or bipolar disorder have more of an important signal-sending brain cell than those without the condition. The scientists, from the University of Michigan, say their discovery backs up previous theories that manic depression is a biological as opposed to a psychological condition. They also suggest that their finding may explain why it appears to run in families. Manic depression is marked by wild, cyclical mood swings. It usually starts when a person is in their late teens or early twenties and affects men and women equally.

Manic Depression and The National Service Framework.
The national strategic framework for mental health states that "one person in 250 will have a psychotic illness such as schizophrenia or bipolar affective disorder." It goes on to cite bipolar affective disorder as one of the 10 leading causes of disability worldwide in adults aged 15-44 How surprising then that the service needs of people with bipolar affective disorder receive no specific consideration in this major policy document. There are four reasons why the national service framework should have given special consideration to the needs of people with bipolar affective disorder. First, most patients with bipolar affective disorder have the potential, with optimal treatment, to return to normal function and contribute to the economy. With suboptimal treatment, however, many will have a poor long term outcome and needlessly become a burden to families and society. Yet there is evidence that treatment is generally suboptimal. For example, a survey (n=1004) of members of the Manic Depression Fellowship, a British organisation for people with bipolar affective disorder, found that only 19% were in full time work, although 40% were graduates and 69% wanted to work. Longitudinal observational studies suggest that the very high lifetime prevalence of suicide attempts in people with bipolar affective disorder (50%3) could be reduced by maintenance drug treatment and adequate treatment of depression and comorbid alcohol and drug abuse.

Manic Depression and the New Year
by
Dr. Peter Melgaard Thompson
Most scientists studying Bipolar Illness agree that this is genetic illness. That is, it runs in families and is passed on though the genes. It has an incidence of about 1%, or 1 out of 100 people in the general population. In families with this genetic problem the incidence is much higher. In identical twins, if one twin has Bipolar Illness, then there is 70% chance that the other identical twin will also have it. As in Major Depression there are very good treatments, in this case not with antidepressants but with mood stabilizers. The oldest and most studied is lithium. More recently, the anti-seizure medications have been shown to have anti-manic properties. In most cases these medications eliminate manic behavior within two weeks and greatly reduce the possibility of having another mania. Lithium is a salt, and is usually taken 2-3 times a day. It is generally a safe medication with little long-term side effects. On the short-term side there are many side effects. The most life threatening is overdose or dehydration. This is because high blood levels can be toxic to the heart. Patients on lithium should have their blood levels checked weekly to every other week. The most common side effects are nausea, weight gain, acne, and thirst. The other group of medicines that are effective in treating mania are the anti-seizure medications. The best studied of this group are carbamazepine (Tegretol) and valproate acid (Depakene or Depakote). These medications seem to work best in patients that cannot tolerate the side effects of lithium or where lithium is ineffective. This group tends to be a subgroup of bipolar patients - the rapid cyclers and dysphoric manics. A rapid cycler is a bipolar patient who goes through four complete cycles a year (manic, depressed and normal mood). The patient with dysphoric mania is characterized by having symptoms of both mania and depression at the same time. As with lithium, weekly or bimonthly monitoring of blood levels are required. The most common side effects are nausea and weight gain.

Manic Depression and Creativity
Many recognized geniuses had creative capacities that were driven by bouts of manic intensity followed by the depths of mind-numbing despair. From Plato, who originated the idea of inspired mania, to Beethoven, Dickens, Newton, Van Gogh, and today's popular creative artists and scientists who've battled manic depression, this intriguing work examines creativity and madness in mystery, myth, and history. Demonstrating how manic depression often becomes the essential difference between talent and genius, Hershman and Lieb offer valuable insights into the many obstacles and problems this illness poses for highly creative people. Lieb critiques the wave of new books on depression as well as those on creativity to determine how far we have come in our understanding of this complex illness. The authors also explode the myth that suffering is essential to creativity. Guides for the manic depressive are suggested to reduce emotional pain and personal problems while increasing productivity. Julian Lieb, a psychiatrist in private practice and former director of the Dana Psychiatric Clinic at Yale-New Haven Hospital, and D. Jadlow Hershman authored
