Preface
from the Surgeon General U.S. Public Health Service We know more today about how to treat mental illness effectively and appropriately than we know with certainty about how to prevent mental illness and promote mental health. Common sense and respect for our fellow humans tells us that a focus on the positive aspects of mental health demands our immediate attention. Promoting mental health for all Americans will require scientific know-how but, even more importantly, a societal resolve that we will make the needed investment. The investment does not call for massive budgets; rather, it calls for the willingness of each of us to educate ourselves and others about mental health and mental illness, and thus to confront the attitudes, fear, and misunderstanding that remain as barriers before us. It is my intent that this report will usher in a healthy era of mind and body for the Nation.

Editorial: Recent Developments in Atypical Antipsychotic Medications During the 1990s there have been some dramatic advances in the treatment of schizophrenia, and the publication of the Schizophrenia Patient Outcomes Research Team (PORT) study ( Schizophrenia Bulletin (Vol 24, No. 1, 1998)) is an opportunity to point out where we believe the "state of the science" is heading. Just as the serotonin re-uptake inhibitor (SRI) class of antidepressants has largely replaced the older and more problematic tricyclic antidepressants (based in large part on safety and side-effect considerations), a shift in the treatment of schizophrenia is now taking place. Some quite recent studies (since the PORT data were collected) indicate that newly available treatments for schizophrenia may offer substantial clinical advantages over the older standard antipsychotic (or "neuroleptic") medications.
Because these medications have been available for only a short time, there is relatively little scientific literature on the subject, but the previous issue of Schizophrenia Bulletin (Vol. 23, No. 4, 1997) contains several relevant articles. Sheitman et al. (1997) note that

Mental health and the law interact in numerous ways. The term "competence" signifies a legal conclusion that an individual is capable of entering into a binding contract, transferring assets, or participating in a legal proceeding. The provision of mental health services is regulated and, to some extent, publicly supported. Legal standards surround the process by which those who are mentally ill can be forced, against their will, to receive treatment. Statutes for involuntary commitment whether denominated civil or criminal are subject to the due process clause of the 14th Amendment . This is because involuntary commitment severly infringes on a person's right to be free from governmental restraint and the right to not be confined unnecessarily. Courts have held that such statutes must bear some reasonable relation to the purpose for which the individual is committed. Finally and most conspicuously, the criminal justice system has, of necessity, to address issues of responsibility, appropriateness of trial and treatment in the light of mental health considerations.
