The Medicare program maintains exhaustive records of hospitalizations, which makes it possible to define the patterns of use of hospital care. When Medicare enrollees are admitted to hospitals, the program’s records identify both the patients’ places of residence (by ZIP Code) and the hospitals where the admissions took place (by a unique numerical identifier). These files provide a reliable basis for determining the geographic pattern of health care use, because research shows that the migration patterns of patients in the Medicare program are similar to those for younger patients. Medicare records of hospitalizations were used to define 3,436 geographically distinct hospital service areas in the United States. In each hospital service area, most of the care received by Medicare patients is provided by hospitals within the area. There are 109 of these hospital service areas in Michigan. The maps in this section show the location of each of these areas. Hospital service areas have been further aggregated into hospital referral regions, based on the pattern of use of cardiac surgery and neurosurgery. The maps also show the hospital referral regions to which the hospital service areas belong. There are 15 hospital referral regions in Michigan.

    Multi-layered Economic Restructuring in an Old Industrial Region: The Pittsburgh Transition
    Sabina Deitrick, Graduate School of Public and International Affairs, University of Pittsburgh, Pittsburgh, Pennsylvania, USA The restructuring of regional economies, particularly in older industrial regions, presents a complicatead version of a post-industrial transformation. Researchers have examined the relation between changes in the structure of production and regional development trends through the post-Fordist transition. Others have studied new propulsive forces in regional development through innovative local milieux. Transition, however is uneven across space. This paper examines theoretical reasons for the economic transition in Pittsburgh, Pennsylvania, through an analysis of employment trends over the past decade. The findings show that, despite a quite remarkable economic turnaround in the late 1980s in the wake of severe steel-related employment losses, the region's pace has slowed considerably in the 1990s. The trend has shifted from a 1980s model of successful post-industrial transition to laggard growth in the 1990s. The article examines reasons for this change by adopting a multi-layered view of transition in older industrial regions.

    ven in the best of times, that is the late lamented 1990s, Detroit was at best a laggard, a city that only slightly felt the heady urban renaissance that swept the nation. Now, in the aftermath of the Sept. 11 disaster and amid economically recessionary conditions, things are going to get tougher not just for Detroit, but for all American cities.
    This new situation will fall now on the watch of new Mayor Kwame Kilpatrick. The first impact will be budgetary. With the new federal emphasis on military spending, and the return of the budget deficit, it seems unlikely that Detroit will garner much more aid from Washington in the coming year. The state of Michigan, reeling from recession, will also not provide additional help.
    Under such circumstances, even the most talented new man in City Hall would not be able to turn around things. To his credit, former Mayor Dennis Archer managed to reverse many of the worst aspects of Coleman Young's destructive policies, but the fundamentals in terms of unemployment, job creation, crime remain pretty much the same as before. While its crime rate is down, Detroit still ranks high nationally in several categories of violent crime. The population, nearly two million a half century ago, is less than half that today.

American Cities THE CENTRAL CITY THE SUBURBS THE NEW URBANISM ASSESSING THE AMERICAN CITY We're coming, finally, to a sequence of four portraits of the world's humanized landscapes. That's what human geography is finally about. You could say that we're looking at pattern now, rather than process. Or you could say that we're looking at the visible consequences of all the activities we've been tracing. I want to begin with American cities, because they're the places we know best. What comes to mind with this topic? One thing is downtown, that cluster of skyscrapers that immigrants first saw from the decks of liners coming into New York's harbor. Now we mostly see downtown through a car or plane window, but it still grabs our attention. Around it, there is the huge penumbra of suburbia: freeways, arterial streets, curvilinear residential streets, commercial strips, a carpet of houses with backyard pools, and a scattering of bigger buildingsshopping centerss, college campuses, hospitals, stadiums, warehouses, and factories. Those are the tangibles. There's something else, too. Maybe it's excitement. Maybe it's fear. Maybe it's a sense of a world with nothing worth doing or seeing. Maybe it's some of each.
