The parent guardian or custodian of a compulsory-school-age child described in this subsection or the parent guardian or custodian of a compulsory-school-age child attending any nonpublic school or the appropriate school official for any or all children attending a nonpublic school shall complete a "certificate of enrollment" in order to facilitate the administration of this section. The form of the certificate of enrollment shall be prepared by the Office of Compulsory School Attendance Enforcement of the State Department of Education and shall be designed to obtain the following information only:

Parent Centers train and inform parents and professionals, help families obtain appropriate education and services for their children with disabilities, work to improve educational results for all children, resolve problems between families and schools or other agencies and connect children with disabilities to community resources that address their needs. Parent centers are funded by the U.S. Department of Education under the Individuals with Disabilities Education Act (IDEA). Each state has at least one parent center, and states with large populations may have more. There are approximately 100 parent centers in the U.S. Topical Areas IDEA 97, Rights and Responsibilities, Procedural Safeguards, Early Childhood, Transition (these are just a few of the topics covered by parent centers)
