FDA panel divided over new diet drug
March 13, 1998 Web posted at: 11:44 p.m. EST (0444 GMT) GAITHERSBURG, Maryland (CNN) A federal advisory panel could not reach a decision Friday on whether the Food and Drug Administration should approve the use of Xenical, a new anti-obesity drug, because of concerns of a possible link between the drug and breast cancer. The 5-to-5 vote by the FDA advisory panel means that it will be up to the FDA to make a final decision, which is likely to come in May. Xenical works by preventing the body from absorbing about one-third of the fat in the food a person eats. In a one-year trial of the drug, the average patient lost 10 percent of his or her body weight. Linda Ciampa reports. The drug does have uncomfortable side effects. The more fat people eat while taking Xenical, the more likely they are to suffer cramping and diarrhea. Still, doctors think Xenical, while not for the casual dieter, could be a short-term crutch for the truly obese so that they can change bad eating habits and start an exercise program.
Concerns about breast cancer
But the advisory panel had questions about the drug's possible link to breast cancer, not Xenical's side effects.

While significant progress in health, nutrition and quality of life have been achieved in several countries of the region, economic and political conflicts have exacerbated the nutrition situation in several others. According to the Regional database of 1997, 17.3 million (24.9%) children are underweight, 23.9 million (34.3%) are stunted and 5.2 million (7.5%) are wasted in the countries of the Region. It is reasonably believed that severe, acute protein energy malnutrition is a problem in countries with complex emergencies (e.g.: Somalia, Sudan) or those encountering economic and political sanctions (e.g.: Iraq, Afghanistan).

Self-Discovery Through Obesity
by Deanna Waggoner I t's Monday morning, and your alarm clock goes off. You feel more tired than usual. As you get up, your head feels tight, your muscles achy, and your throat is sore. You stumble to the bathroom, pull out the thermometer and take your temperature... 101 degrees! You have the flu. So you immediately look at yourself in the mirror and say to yourself: "You idiot, look what you've done to yourself. You have a fever, how stupid could you be? You are only doing this for attention, a way to get out of work. You're pathetic!" Right? Of course not. You take care of yourself. You rest. You may see a doctor. You drink a lot of fluid. You nurture yourself back to health. If you're holistic, you recognize that fever is a natural defense mechanism to fight off the virus or bacteria that is attacking your body. You may even thank your body for doing such a good job. We are conditioned in this society to only think of the tangible, physical aspects of the world around us, and so the main emphasis regarding obesity for many years has been food and diet.

WHO/FAO release independent Expert Report on diet and chronic disease
Less saturated fats, sugar and salt, more fruit and vegetables and physical exercise, needed to counter cardiovascular diseases, cancer, diabetes and obesity
A diet low in energy-dense foods that are high in saturated fats and sugars, and abundant in fruit and vegetables, together with an active lifestyle are among the key measures to combat chronic disease recommended in an independent Expert Report prepared for two UN agencies. The report, commissioned by the World Health Organization (WHO) and the Food and Agriculture Organization (FAO), from a team of global experts, aims to identify new recommendations for governments on diet and exercise to tackle the ever increasing number of people who die each year from chronic diseases. The burden of chronic diseases – which include cardiovascular diseases, cancers, diabetes and obesity – is rapidly increasing worldwide. In 2001, chronic diseases contributed approximately 59% of the 56.5 million total reported deaths in the world and 46%of the global burden of disease. The Report includes advice on ways of changing daily nutritional intake and increasing energy expenditure by:
  • reducing energy-rich foods high in saturated fat and sugar;

What is obesity?
Obesity is generally defined as the condition of weighing 20% or more over your ideal weight. You can find the ideal weight for your height and sex from the weight chart below. Who is affected by obesity? Forty million Americans are obese. Forty percent of women between the ages of 40 and 49 are at least 20% over their ideal weight. Obesity increases as people age. Why is it important to control obesity? Obesity is a serious condition because it increases your risk of having major illnesses, such as stroke heart disease diabetes some types of cancer , such as colon, breast, and uterine cancer and osteoarthritis (the stiff, sore joints that you can get as you get older). Obesity makes it harder for your health care provider to give you a good physical exam. Looking at the inside of your body with X-rays and other types of scans, as well as surgery, may also be more difficult. What causes obesity?

What is gross obesity?
Gross obesity is defined as weighing more than twice your ideal weight or being more than 100 pounds overweight. What causes obesity? The reasons for obesity are not clear. Although obesity occurs when the number of calories consumed is greater than the amount of energy spent, not all obese people overeat. They may have a slower rate of metabolism, the amount of energy needed to maintain the basic body functions when at rest. Obese people also use up fewer calories because it's harder for them to be physically active. Emotional problems, such as depression, anger, and anxiety can occasionally be a cause. Emotional problems can contribute to the obesity or can result from it. Is obesity hereditary? Heredity is a contributing factor. Children who have obese parents are 10 times more likely to become obese adults than children who have parents of normal weight. There is also an environmental factor because unhealthy eating habits may be reinforced in families. A few obese people have an imbalance of hormones.

Several studies have found a strong link between obesity and time spent watching TV. This isn't surprising.

Too much TV is bound to prevent kids from developing the skills and love of sports that make physical activity so enjoyable.

Obesity is a major contributor to the development of type II diabetes. Development of both obesity and diabetes results from an interaction of predisposing genes with environmental factors. Inbred strains vary in their susceptibility to high fat and/or sucrose feeding with strains like C57BL/6J, C57BLKS/J, and AKR/J showing high susceptibility to development of moderate obesity, hyper-glycemia, and insulin resistance C57BL/6 male mice are rapidly becoming the most commonly used mouse model for diet induced obesity (DIO). Obesity Treatments: What Works, What Doesn't
Obesity research has proven beyond a doubt the potential health risks of being overweight. Heart disease, diabetes, hypertension, strokes: the list of health problems is both frightening and discouraging. Frightening, because so many of these diseases are life-threatening or debilitating. Discouraging, because while obesity research tells us a lot about the risks of being overweight, it tells us very little about how to lose weight.
One Size Never Fits All
Obesity treatment is a complicated matter: what works for one person may not work for another. Once you factor in lifestyle, genetics, and personal weight and health issues, it's easy to see that treatment has to be tailored to the individual.
And therein lies part of the impediment to successful obesity treatment: so many weight loss programs presume that everyone's the same. Programs greatly over-simply the issue when they assume that the needs of someone whose weight has genetic origins are the same as someone who suffers from a binge eating disorder. Unfortunately, all too many weight loss programs do just that.
The other great barrier to weight loss is the simple fact that losing weight is very hard work. Obesity research may scream the virtues of an active lifestyle, but for many truly obese people, exercising hard enough to burn calories is uncomfortable, painful, and in some cases, downright dangerous.
