News: February 2002, a settlement proposal for nearly $1 billion was reached with Sulzer. Final details on the settlement, including the compensation for each person, are still under negotiation. Do you have a recalled hip joint?

Sulzer Medica disclosed that a "mineral-oil based lubricant" was found on the surface of the recalled Inter-Op® shells. In some patients, this lubricant has prevented proper bonding of the prosthesis with the patient's own surrounding bone and tissue causing joint instability and severe pain. Sulzer had reported that 129 people had undergone required revision surgery by mid-January, 2001. However, that number has now increased to 573 as of March 1, 2001.
S ulzer Orthopedics Inc. has now received adverse reports concerning one of its artificial knee parts. Though the manufacturer is not recalling the product, it did withdraw it from the market in March due to a problem with one of the parts of the knee prosthesis called the tibia baseplate. About 1,300 people in the United States have received the knee part so the potential problem with the knee part is far smaller in scope than the problem with the Inter-Op hip prosthesis.

Materials and Surface
Bone ongrowth on an SL-PLUS stem at 25x magnification. Stiffness of the SL-PLUS stem in comparison to a canal filling stem. Bone has the ability to remodel by altering its size, shape, and structure to meet the mechanical demands placed on it. This phenomenon, in which bone gains or loses cancellous and/or cortical bone in response to the level of stress and strains sustained, is summarized as Wolff's law, which states that bone is laid down where needed and resorbed where not needed. The biological mechanisms that produce these effects are still not completely understood but involve electrical, chemical, and hormonal mechanisms as well as mechanical processes at both the cellular and organ level. Typically, all load is handled exclusively by bone. Following hip arthroplasty, load is now shared by the prosthesis, reducing load on the bone. If this reduction is high, it can induce atrophic remodeling (according to Wolff's law) and lead to decreased bone density and fractures. The phenomenon is called "stress shielding." Description of the Problem: Reports indicate that the humerus (shoulder bone) is commonly broken following trauma or during race training. However, it is not common for this bone to break during recovery from general anesthesia. In a recent study investigating the use of the humerus as a potential site for bone graft harvesting, the reduction of the bone strength following the removal of graft material was called into question. Researchers want to determine if the risk for catastrophic fracture is in any way increased by the removal of bone graft material.
Study Objectives: Determine if taking bone graft material from the top of the humerus weakens it enough to cause fracture during recovery from general anesthesia.
