A male patient age 25, 6ft 2inches tall, normal weight, presented with pain to the right of the thoracic spine radiating around to sternum which he described as very acute at times particularly when tired. The pain spread as a band around chest to sternum and then would slowly fade away. His job as a nurse in the intensive care unit involved lifting. He worked night shifts - when he was particularly aware of the pain. The pain eased on rest. Consultation and history taking gave a picture of a healthy individual who exercised by walking his dog, ate sensibly and was in a good relationship. There was no medical history of note. He has not consulted his doctor about this problem as past experience has shown him that all he will be given is tablets. He has come on recommendation of a work colleague. Onset had been insidious and he could not remember exactly when the pain began but thought he could relate it to lifting someone a week or so previously - though he admitted he was not as aware of his biodynamics on night duty. The pain was now getting worse and he asked how osteopathy worked.

OSTEOPATHY Osteopathy is an established, recognised system of diagnosis and treatment that lays its main emphasis on the structural integrity of the body. It is distinctive in the fact that it recognises much of the pain and disability we suffer stems from abnormalities in the function of the body structure as well as damage caused to it by disease. Osteopathy uses many of the diagnostic procedures used in conventional medical assessment and diagnosis. Its main strength, however, lies in the unique way the patient is assessed from a mechanical, functional and postural standpoint and the manual methods of treatment applied to suit the needs of the individual patient. Osteopaths use their hands both to investigate the underlying causes of pain and to carry out treatment using a variety of manipulative techniques. These may include muscle and connective tissue stretching, rhythmic joint movements or high velocity thrust techniques to improve the range of movement of a joint. Gentle releasing techniques are often used, particularly when treating children or elderly patients. The cranial approach also uses very gentle techniques in which the osteopath's highly trained sense of touch is used to identify and correct mechanical disturbances and limitations, both in and around the joints for the skull, and throughout the body.

OSTEOPATHY IN THE CRANIAL FIELD
William Garner Sutherland DO Dr. Sutherland (1873-1954) was a student of Andrew Stills (~1900) who took Dr. Still's admonition to "dig deeper" very seriously. As a student Dr. Sutherland had an inspiration that the bony cranium was capable of respiratory motion. He initially did not understand its implications, but could not let the idea go. Throughout his years of practice, with diligent research and study, he continuously developed a revolutionary expansion of the osteopathic concept. Dr. Sutherland called his discovery "Osteopathy In The Cranial Field" (OCF), implying that he did not create anything new; he had simply applied osteopathic thinking and principles to the cranium. Though it did not fully emerge until much later in Sutherland's life, at the core of his new understanding lies the concept of PRIMARY RESPIRATION. The definition of the Primary Respiratory Mechanism (PRM): Primary : it is a system that comes "first". It underlies all of life's processes and gives dynamism, form, and substance to all of anatomy and physiology. Respiratory : It is the spark that gives rise to the breath, as it moves through the tissues. It is the foundation of metabolism. It has both an inhalation and exhalation phase.

OSTEOPATHY AND POSTURE Osteopathic treatment concentrates on the relationship between the structure of the body - the skeleton, muscles, ligaments and connective tissue - and the way in which the body moves and functions. The literal meaning of the word osteopathy is 'bone disease' - a rather unfortunate term that does nothing to evoke the great benefits of this safe, natural system of diagnosis and treatment. To an osteopath, the body functions as a complete, working system, so any problems affecting its structure, upset the balance of our general health. For this reason, an osteopath will not simply want to ease the pain and stiffness in your neck, but also want to know what is causing that pain, which could be anything from physical injury to mental anxiety. The therapy was devised in the 19th century by an American doctor, Andrew Taylor Still. He became disillusioned with medicine when three of his children died of viral meningitis. Still sought an alternative in the philosophy of Hippocrates, who claimed that the 'cure of disease lies within the body'. An interest in osteopathy developed through his belief that tension in muscles and misaligned bones places unnecessary strain on the body. This strain can be caused by any number of things, such as physical injury, bad posture, or by emotions such as anger and fear. Whatever the cause, Still believed it could be eased by adjusting the framework of the body, so that all the systems within it would run smoothly and the body could heal itself. Of all the bone structures in the body, Still considered the vertebrae of the spine to be the most important. Still's reasoning was simple: the spine protects the spinal cord, a major part of the nervous system, and the nervous system penetrates every area of the body, controlling voluntary and involuntary movement and registering every sensation, from the most delicate smell, to tortuous pain. Still believed that anything interfering with the nervous system could resonate in every area of the body.

Osteopathy is a medical practice based on the theory that diseases are due chiefly to loss of structural integrity which can be restored to harmony or equilibrium by manipulation. The manipulation allegedly allows the body to heal itself. Osteopaths use manipulation for diagnosis, treatment, and prevention of disease. Andrew Taylor Still (1828-1917), a Civil War surgeon in the Union army, is credited with discovering osteopathy as an alternative to the medical practices common in his day, practices which failed to save his three children from spinal meningitis. Still became convinced that he could cure diseases by shaking the body or manipulating the spine. In his autobiography, he says he could "shake a child and stop scarlet fever, croup, diphtheria, and cure whooping cough in three days by a wring of its neck" (Barrett) . He also advocated clean living, including abstinence from alcohol and medically prescribed drugs. Surgery was to be avoided, if possible. Today, D.O.s (doctors of osteopathy) complement manipulation with standard medical methods of diagnosis and treatment, including recommending drug therapy and surgery if appropriate. D.O.s have four years of medical training at a college of osteopathic medicine and do a one-year internship in primary care. Some continue their education in an area of osteopathic specialization. Nevertheless, there has still not been scientific validation of Still's theory of shaking and manipulating to remove obstructions.
