  • Health Chat TORONTO (CP) As a pitcher, Bill Risley of the Toronto Blue Jays has keen balance. Risley's young son Kaleb, on the other hand, has trouble with balance even with splints. Kaleb has spina bifida , a neural-tube defect that will affect about one in 750 children born in Canada this year. Risley and his wife Brenda consider themselves lucky even though they were thrown yet another medical curve. Risley has a history of back problems. A few years ago, their daughter Sage scalded her throat when she put her mouth over a steaming pot of tea. The relief pitcher said an ultrasound on his pregnant wife revealed Kaleb's spine wasn't forming properly. But the early diagnosis gave the couple time to prepare for the challenges ahead.

    Spina Bifida
    Definition:
    Spina bifida results from the failure of the spine to close properly during the first month of pregnancy. In severe cases, the spinal cord protrudes through the back and may be covered by skin or a thin membrane. Surgery to close a newborn's back is generally performed within 24 hours after birth to minimize the risk of infection and to preserve existing function in the spinal cord.
    Because of the paralysis resulting from the damage to the spinal cord, people born with spina bifida may need surgeries and other extensive medical care. The condition can also cause bowel and bladder complications. A large percentage of children born with spina bifida have hydrocephalus, the accumulation of fluid in the brain. Hydrocephalus is controlled by a surgical procedure called "shunting" which relieves the fluid build up in the brain by redirecting it into the abdominal area.     Spina bifida (SB) a type of myelodysplasia is the most common neural tube defect. SB caused by the failure of the fetus's spine to close properly during the first month of fetal development. The Spina Bifida Association of America reports that spina bifida is the most common permanently disabling birth defect.48 Spina bifida may be categorized into 2 type (some texts include spina bifida cystica in which a CSF filled sac protrudes f rom the spinal column. Spina bifida aperta includes 2 main types: myelomeningocele and meningocele. Myelomeningocele is the severest form in which the spinal cord and the meninges protrude from an opening in the spine. Meningocele is a less severe form in which the only the meninges and nonfunctional nerves protrude in a sac wall. Spina bifida occulta, the mildest form, is typically only a vertebral defect in which the vertebral arches do not fuse.
    Epidemiology:
    Spina bifida affects 0.5 to 1 out of 1,000 infants born in the United States. The group with the lowest incidence is African blacks with 1 in 10,000. There is a sibling

    GLOSSARY OF GENETICS TERMINOLOGY Alpha-fetoprotein - The predominant protein present in fetal blood. Small amounts are present in the maternal circulation during pregnancy, and the values change as the fetus grows. Abnormally high or low values are associated with increased risks of spina bifida and Down syndrome, respectively.
    Amniocentesis - A procedure for prenatal diagnosis in which about one tablespoon of amniotic fluid surrounding the fetus is withdrawn from the amniotic sac using a very fine needle. Genetic amniocentesis is performed at The Genetics Center. Continuous ultrasound guidance during the amniocentesis makes the 60 to 90 second procedure as safe and painless as possible.
    Autosomal recessive / dominant - Patterns of inheritance in which females are as likely to be affected as males. In recessive disorders, one gene (of the pair) needs to function properly to prevent the disorder. For a dominant disorder, one gene functioning abnormally is sufficient to cause the disorder.

    Run aids spina bifida The Crusader/Kelly Gerrity WALKING FOR DYLAN - Sophomores Sarah Clark and Karen Littlefield and senior Shana Hull enjoy the warm weather during the Dylan's 5K run/walk for spina bifida on Sunday. By Kim Hollenbush
    Staff Writer Susquehanna hosted the second annual Dylan’s Run at Nicholas A. Lopardo Stadium Sunday, April 7 at 1:30 p.m.
    The 5K run/walk raised awareness and money for the fight against spina bifida. Dylan’s Run benefited The Spina Bifida Association of the Delaware Valley.
    “I’m really glad to see that they’re raising awareness of spina bifida, it means a lot to me because my older brother was born with spina bifida and hydrocephalus,” Sarah Stout, junior, said. “Knowing that this is one of the most common birth defects, I think that people need to be more educated about spina bifida, and I think that Dylan’s Run is a great way to make people more knowledgeable.”
    It is estimated that the 5K run/walk raised about $1800 for The Spina Bifida Association of the Delaware Valley. Seventy-five people participated in the race. Registration for the race cost $12. Teams of eight or more people registered for $10 per person. Each participant received a free T-shirt.

Genetics/Birth Defects Spina Bifida Audio File Spina bifida is a birth defect caused by failure of the lower spine to close properly during the first month of pregnancy. The severity of the defect can range from so mild that it causes no impairment to so severe that the child is unable to walk and has no bladder or bowel control. The degree and type of disability depends upon which nerves are affected and how much. The outlook for each child depends upon the number of abnormalities and their severity. Most children with spina bifida who get proper care live well into adulthood. Spina bifida is a neural tube defect. These are serious birth defects in which there is incomplete development of the brain, spinal cord and/or their protective coverings. Neural tube defects tend to run in families and are thought to result from a combination of genetic and environmental factors. Spina bifida has been linked to a lack of the B vitamin folic acid in early pregnancy. Many cases of spina bifida could be eliminated if all women of childbearing age took 0.4 mg of folic acid daily before becoming pregnant. A health care provider can usually detect spina bifida during pregnancy. If high levels of a certain protein are present in the mother’s blood, other tests can be done to identify and confirm the defect.

  • The development of excess fluid in the ventricles (fluid spaces) of the brain. this is treated by placement of a shunt to drain the excess fluid. 80-90% of babies with spina bifida develop hydrocephalus. Reason for shunt placement:
  • Allow for normal brain development Allow for normal head growth Prevent progressive hydrocephalus
  • Placement of a shunt does not repair brain damage that may have already occurred due to the presence of congenital hydrocephalus nor bleeds that have occurred as a the result of prematurity or birth trauma.
