A microdiscectomy is performed on patients who have disc herniations impinging on the nerve root that can be easily reached and treated. An incision is made in the patient's back in the midline, and it is approximately 1 ½ to 2 inches long. Magnification as well as fiber optic lighting and use of a C-arm minimize the incision needed to remove the disc. This procedure can be done as an outpatient procedure or as an overnight stay. Unfortunately, this type of surgery, which is relatively quick and easy, is not the only type of surgery that is needed for patients with back injuries. Often patients will have a second herniation after the disc has been removed. This then requires a fusion.

Federal Agency to Develop Practice Guidelines for Low Back Disorders
AHCPR Forming Clinical Practice Panel
One of the tasks of the Agency for Health Care Policy and Research (AHCPR) is to "facilitate the development of practice guidelines and measurements of quality care" (please see "Chiropractic Cost Comparisons Presented at AHCPR Conference" in the March 1, 1991 issue). The U.S. Congress created the AHCPR within the Department of Health and Human Services in an effort to improve health care and reduce costs. To that end, the agency seeks to ascertain what types of treatment are most effective in terms of costs and results. Congress has allocated the AHCPR a very large annual budget, part of which will go towards the sponsoring of research and panels that will develop practice guidelines. Ultimately, this endeavor will dictate what types of treatment will be acceptable and reimbursable for specific ailments. Fortunately, within the chiropractic profession, a few individuals and a number of chiropractic organizations are in-step with Congress in the development of practice guidelines. That is why the Conference for the Establishment of Guidelines for Chiropractic Quality Assurance and Standards of Practice (the Chiropractic Quality Assurance Conference) has been formed and sponsored by almost every major chiropractic organization in North America (please see "Chiropractic Practice Guidelines" in the July 19, 1991 issue). There is an important issue you may be over looking in your "analogy"
People with chronic back disorders, or CHRONIC PAIN, have extremely heightened activity of the normal stress response. Pain activates the hypothalamo pituitary adrenal axis (HPA) which is responsible for preparing the system for stress. Chronic activation of this system, due to the chronic pain, could actually result in depression. I find it IRONIC that doctors are so unwilling to treat the pain, giving too little pain prescriptions, but wind up giving anti-depressants for the depression which could have been prevented in the first place.

Contact Us Low-Back Musculoskeletal Disorders Over 40 recent articles provided evidence regarding the relationship between low-back disorder and the five physical workplace factors that were considered in this review. These included (1) heavy physical work, (2) lifting and forceful movements, (3) bending and twisting (awkward postures), (4) whole-body vibration (WBV), and (5) static work postures. Many of the studies addressed multiple work-related factors. All articles that addressed a particular workplace factor contributed to the information used to draw conclusions about that risk factor, regardless of whether results were positive or negative. The review provided evidence for a positive relationship between back disorder and heavy physical work, although risk estimates were more moderate than for lifting/forceful movements, awkward postures, and WBV. This was perhaps due to subjective and imprecise characterization of exposures. Evidence for dose-response was equivocal for this risk factor. There is strong evidence The review provided evidence that work-related awkward postures are associated with low-back disorders. Results were consistent in showing positive associations, with several risk estimates above three. Exposure-response relationships were demonstrated. Many of the studies adjusted for potential covariates and a few examined the simultaneous effects of other work-related physical factors. Again, it appeared that lifting and awkward postures both contribute to risk of low-back disorder.

Back aches are the most common reason for doctor visits, after cold and flu symptoms. Fifty percent of all patients who suffer from an episode of low back pain will have another occurrence within one year. In the vast majority of cases back pain is caused by the irritation of a nerve root near the spine, not by problems with the muscles, ligament or bone. A nerve that travels from the spinal cord through the openings between the bones of the spine gets pinched or irritated, the surrounding muscles tense up and the patient experiences low back pain. Low back pain is widespread in our society, but the good news is that in most instances the pain ends within a few days. More serious cases of back pain are treated with anti-inflammatory medication, physical therapy and muscle relaxants. Surgery, a common treatment a generation ago, is now considered necessary for only a very small percentage of back pain patients.

The back is made up of 33 individual bones called vertebrae, separated by shock - absorbing disks. The spine is held in place by a large number of muscles and ligaments. By acting together, they give the spine the ability to bend and twist. The spine also protects the spinal cord and acts as a distribution center for the nerves that run between the brain and the other parts of the body. Anatomically, the spine is an unstable structure. We create the illusion of stability by using muscles groups in the trunk to keep the back stable. If these muscle groups are out of condition, we run the risk of injury from one - time exertions that are beyond our capacity or from prolonged use of the muscles at a slightly elevated level.
Typical back disorders include:
Strains and sprains are damage to the tendons and ligaments caused by one - time exertions such as lifting or carrying heavy objects. These can lead to very noticeable back pain, but the pain usually begins to subside within a few days if properly addressed and managed.
