The Basics What is it? Benign prostatic hyperplasia (BPH) is a nodular, irregular enlargement of the prostate, the walnut-size gland located just below the bladder in men, which produces about 30 percent of the fluid portion of semen. Because the prostate surrounds the urethra (the passageway through which urine empties from the bladder), enlargement of the prostate may eventually constrict the urethra and thus interfere with urination. An enlarged prostate may also cause the muscular bladder wall to thicken, as stronger contractions are necessary to push urine through a narrowed urethra. Increased thickness of the wall of the bladder can reduce its ability to store urine and can result in frequent need for urination and sudden strong urges to urinate (urgency). BPH is common, and its incidence increases with age: Evidence of BPH is present in over 50 percent of men by age 60. There is no evidence that BPH leads to prostate cancer; however, symptoms of both disorders are similar, and it is possible to have BPH and prostate cancer at the same time. BPH responds well to treatment.

What is benign prostatic hyperplasia (BPH)? BPH (also referred to as benign prostatic hypertrophy) is a condition in which the prostate gland becomes very enlarged and may cause problems associated with urination. BPH can raise PSA (prostate-specific antigen) levels two to three times higher than the normal level. An increased PSA level does not indicate cancer, but the higher the PSA level, the higher the chance of having cancer. Some of the signs of BPH and prostate cancer are the same, however, having BPH does not seem to increase the chances of developing prostate cancer. A man who has BPH may also have undetected prostate cancer at the same time or may develop prostate cancer in the future. Therefore, the National Cancer Institute and the American Cancer Society recommend that all men over 50 consult their physicians about having a digital rectal and PSA examination once a year to screen for prostate cancer. How does BPH occur?

Benign Prostatic Hyperplasia
Enlargement of the prostate affects about one third of men over 50 and can cause distressing urinary symptoms. The progress of benign prostatic hyperplasia (BPH) is unpredictable, but only a minority of men deteriorate rapidly and some will improve spontaneously. Many men willingly tolerate mild symptoms of BPH. Care should be taken not to overtreat men who are not too bothered by their symptoms. For the majority of men whose symptoms are not unacceptably severe, the condition may be best managed by watchful waiting and simple lifestyle changes. The most effective treatment for severe symptoms is surgery, but about a quarter of men fail to benefit and some end up worse. The most common operation for BPH is transurethral resection of the prostate (TURP). Incision of the prostate (TUIP) is often just as effective, uses fewer resources and is less hazardous; however it is under-used. Drug therapy on average has a small effect on symptoms, but some men may experience significant benefit.

What to Expect at Your Provider's Office Your health care provider will feel your prostate gland directly by putting a gloved finger in your rectum. He or she will also order blood tests and possibly a urine sample. Your health care provider may also ask you to urinate into a device that measures the flow of urine. In intravenous pyelography, your health care provider injects a dye into a vein to make the flow of urine visible on an X ray. In cystoscopy, your provider uses a small probe, passed through your urethra, to directly view the inside of your urethra and bladder. Your penis will be numbed before this procedure.
