    Lifeline, Winter 1995, Vol XIII, No 1, pp 1-2 Treatment for nontropical sprue includes correcting the nutritional malfunction of the small bowel and the deficiency states. Removal of gluten from the diet results in the histological appearance of the intestinal mucosa returning to the normal state. Those conditions represent the need for the clinical gluten-free diet for nontropical sprue (celiac sprue) patients. Gluten-Free Diet. A specific diet that omits the glutamine-bound fraction of protein (glutenin and gliadin) is the clinical treatment of choice for nontropical sprue. In this diet wheat, rye, barley and oats are excluded, since they all contain large amounts of this protein fraction. Buckwheat and millet are also excluded because of lack of reliable information about their glutamine content. The gluten-free diet means that all bakery products and packaged foods must be scrutinized before they are used. Labels must be read carefully because, besides being a basic ingredient, gliadin-containing grains may be added as a derivative when the food is processed or prepared. For example, hydrolyzed vegetable protein may be made from soy or corn, or it may be made from mixtures of wheat, corn and soy. Thus, the need to know the content of hydrolyzed vegetable protein (HVP) before ingestion by celiac patients.

Causes and Risks The exact cause of celiac disease is unknown. There is evidence that the disorder is inherited. (Its incidence is much higher in siblings.) The disorder may be caused by an abnormal immune response to proteins found in grains, particularly gluten and the related protein gliadin. The intestines contain projections (called villi) which normally absorb nutrients. Celiac disease causes the villi to become flattened and lose the ability to absorb nutrients. Weight loss anemia , and vitamin deficiences may occur as a result of the malabsorption (inadequate absorption of nutrients from the intestinal tract).

Gluten is present in many grains including rye, oats, barley, and triticale. Symptoms in infants include diarrhea, slow growth, bloody stools, weight loss, vomiting , and clay-colored stools. Symptoms in adults include abdominal distention, abdominal pain, fatigue, weight loss, bone pain, and bone tenderness. Treatment involves the life-long avoidance of gluten.

    Celiac disease is a malabsorption syndrome. The intestine is not able to absorb vital dietary nutrients from foods containing gliadin, an alcohol-soluble portion of gluten.

    Nutritional considerations
    are important for persons with celiac disease. People with celiac disease are sensitive to cereal protein found in wheat and other grains. But they are more sensitive to the protein in wheat than the protein in other cereals such as rye, oats, and barley. The protein causes a reaction in the person's small intestine that prevents absorption of essential nutrients from the diet.     Celiac disease is a chronic digestive disorder that affects the small intestine. It can begin at any stage of life, from infancy through adulthood. The disease varies in severity from person to person. People with celiac disease have an intolerance or a reaction to gliadin, which is a component of gluten. Gluten and gliadin are found in several common grains.
    What food source is the nutrient found in?
    Gluten is found in wheat, rye, barley, oats, corn, and rice. Gliadin, the offending substance, is not present in corn or rice but is found in the other grains. So, the diet for celiac disease is actually known as gluten-restricted and gliadin-free. People with the disease cannot eat wheat, rye, barley, and oats or any products made with these grains.     MALABSORPTION AND DELINQUENCY A way the hypothalamic choreographer might be deranged is by malabsorption syndrome. If this suggestion is valid it directly leads to some simple therapeutic guidelines and implications for inexpensive and productive research. Malabsorption might result from:
    • constitutional inadequacy; failure of intestinal flora; some variant of celiac disease;(Salvadori, 1976 other food intolerances, especially to cow's milk; toxins - e.g., lead or manganese, or even so common a substance as granulated cane sugar; psychosocial stress.
    Malabsorption would result in:
    • proteinurias caused by toxic processing in the gut and imbalances of protein intake; vitamin dependencies, also resulting from toxic by-products and poor uptake, especially in the new born; lipidosis;
