Although it's not a guarantee you won't get the flu, the vaccination will greatly lower your chances of catching it," said Dr. William Glezen, a professor of molecular virology and microbiology at Baylor College of Medicine in Houston. The best time to get the shot is from the middle of October to the middle of November, because most people get the flu in the winter. In the Northern Hemisphere, the "bug" traditionally arrives in December. It is a highly contagious viral infection of the nose, throat and lungs, and it is one of the most severe illnesses of the winter. It spreads easily from one person to another primarily when an infected individual coughs or sneezes. A new variant of the influenza B virus, B/Hong Kong/2001, showed up last spring. This virus will likely spread again this winter. Fortunately, Glezen said, it is included in this year's vaccine. This year marks the first time parents are being urged to get babies and toddlers vaccinated because influenza sends its tiniest patients to the hospital as often as it does the elderly. But parents need to be aware that unlike the one yearly shot most people need, the first-ever inoculation for young children requires two doses a month apart. Parents shouldn't delay that visit to their pediatrician.

When Gail asked me to write for the Empowering Caregivers Spotlight, my first thought was "why me?" Most of the caregivers with whom I've talked care for elderly parents or spouses, and I take care of our 28-year-old daughter. But then I remembered that "caregiving is caregiving" no matter to whom or how old. We all have something to contribute.
My husband Jerry and I were married on November 22, 1969. I met Jerry at church - the "new girl" in the choir. When we married I knew he had diabetes, and although I watched as he gave himself insulin shots, I never knew much about the disease. Other than that, he was in perfect health.
We were blessed with a daughter (Kaylie) in 1971 and a son (Stephen) in 1978. Kaylie began showing signs of diabetes at the age of four and was immediately put on insulin, which I had to administer to her myself. Giving shots to a four-year-old is not my idea of fun.
But my real "caregiving" experience began in the spring of 1982. Kaylie, who by then was 10, had been sick off and on for about 3-4 months. Each trip to the hospital involved admission and IV hookups. We were home the next night (usually) but realized we didn't test blood sugars as often as we should.

    My son, Will McCarthy, was born a healthy happy baby. On July 23, 1984, he developed Reyes Syndrome and stayed in the hospital for six weeks. He is now classified with Ataxic Cerebral Palsy as a result of Reyes Syndrome in infancy. When it was apparent he was not going to be able to communicate, we started a yearlong battle to get him a Touch Talker™ from Prentke Romich Company. After a year, and many letters from our lawyer who worked free of charge for Will's cause, he was able to "talk." In 1987, Will "spoke" to our governor to get legislation passed for state insurance to buy communication devices for people. Again in 1987, he spoke before a state legislative committee to get the legislation passed. It passed! In 1988, he spoke to Dale Murphey who played for the Atlanta Braves, when Dale was in Chattanooga for a fundraiser. Will has been published in many articles because of his unique way of talking. In 1989, my husband and I started a Handicapped Baseball League at a local park and, in 1998, the park named the field where the league plays for Will and our family. Will "sang" the National Anthem at the dedication. Will rode in a National Walking Horse Show in 1991, in Shelbyville, Tennessee. As he rode around the ring and received a standing ovation, the announcer shared Will's story of his favorite horse. For his tenth birthday, he got to go back stage and meet country music singer Ricky Van Shelton. Will had preprogrammed some questions and the singer was amazed.

    A spirin , that long-term over-the-counter (OTC) pain reliever has become known to reduce risk of a stroke or heart attack or angina (chest pain) in those who have previously experienced any of those conditions; or in those with recurrent blockage after a heart bypass, angioplasty, or carotid endarterectomy. It has been observed that doses commonly recommended for cardiovascular uses have been too high, and that, as a general rule, 50-325 milligrams daily is sufficient - 75-325 for those with angina or previous heart attacks. It is believed by scientists that aspirin's effectiveness for pain and inflammation relief and reducing risk of cardiovascular events lies with its ability to reduce the body's production of prostaglandins. They can cause blood platelets to adhere to each other (form clots), which can cause blood vessel blockages, and eventually heart attacks or strokes. There are, of course, well-known risk factors associated with aspirin therapy, so before starting, consult with your doctor about the risks versus the benefits. The risks are:

    Not that many years ago, back when I was a kid, choosing a nonprescription pain reliever was simple: You could have aspirin or aspirin. Things haven't been so simple, though, since acetaminophen and ibuprofen climbed onto pharmacy shelves. And recently, with the appearance of naproxen sodium, they got even more complicated.
    Which one's which and what are they good for? Basically, the four general types of pain killer are about equally effective. But some do work better than others for certain problems, and their side effects differ significantly. Here's what you should know about these medications, along with a few examples of common brand names to help you in your shopping.
    Aspirin (Anacin, Ascriptin, Bufferin, Ecotrin, Empirin): This is the most economical form of relief for pain and inflammation, and it also reduces fever. Generic brands are widely available and are cheaper yet. Aspirin should not be taken by people with aspirin allergy, asthma, bleeding disorders, gout, or ulcersor by those who are taking blood thinners. Overuse can lead to upset stomach, bleeding in the digestive tract, and ulcers. Children should avoid it because of Reyes syndrome, and women should not take it during the last three months of pregnancy.
    Ibuprofen (Advil, Ibuprin, Motrin IB, Nuprin): Very similar to aspirin in makeup and effects. But you might keep some around for your significant other, because ibuprofen does seem to be more effective than aspirin for menstrual cramps. Side effects, too, are similar to those of aspirin, with the exception that ibuprofen is not associated with Reyes syndrome.

    Fever in the first few years of life can be the initial manifestation of benign as well as life threatening illness. Fever is the most common reason for which children are brought to the emergency department. In order to avoid missing serious conditions associated with fever, the physician must determine which children need further diagnostic evaluation or in-hospital stay and which patients may be discharged for outpatient treatment and follow-up.
    Definition of fever
    In general, fever is considered to be present if the rectal temperature is above 38.3oC, oral temperature is above 37.8oC or axillary temperature is above 37oC. FEVER SYNDROMES Fever with focus
    The majority of febrile episodes in children are associated with infections. The diagnosis is made by identifying the focus: e.g. otitis media, upper respiratory infection, septic arthritis etc.
    Fever without localising signs
    This term refers to the presence of fever in a child in whom a careful history and physical examination fail to reveal a cause. The physician may wait up to one week, before certain investigations are made. However, fever without localising signs also may be due to conditions like urinary tract infections, or prodromal phase of conditions like infectious mononucleosis and Kawasaki disease.

Reye's (pronounced "rye") syndrome is a disease which is believed to be caused by the ingestion of medicines (such as aspirin) that contain salicylate (pronounced "sa(LISS(a(late") that affects all organs of the body, but affects the liver and brain most lethally. It occurs when abnormal accumulations of fat develop in the liver and other organs of the body, along with a severe increase of pressure in the brain. It affects mostly children and teenagers and appears soon after flu-like infection or chicken pox. Reye's syndrome occurs most frequently during the months of January, February and March when influenza is most common, although cases have been reported throughout the year. Reports of the syndrome appeared as early as 1929, but it wasn't until 1963 that the symptoms were first recognized as a distinct disease by R. Douglas Reye, M.D., an Australian pathologist. Dr. Reye suspected that a drug or poison (later identified as aspirin) triggered the disease's development, but there was no proof of such a substance in the homes of the children who acquired the illness, nor did autopsies reveal the presence of any such substance ingested by the child. Reye's syndrome is considered an acute, life-threatening disorder. When Reye's syndrome occurs, the liver begins to swell and fat deposits form. Blood sugar levels drop, high levels of ammonia and acid form in the blood and the brain swells. ASPIRIN IS THE TRIGGER Reye's syndrome is associated with giving aspirin to children with a viral infection. If the disorder is diagnosed and treated early, the child's chance of survival is greatest. KNOW THE SYMPTOMS persistent nausea and vomiting following a viral infection drowsiness, stupor, loss of consciousness or coma delirium, seizures or convulsions DIAGNOSIS AND TREATMENT The symptoms generally start a week after the viral infection. Nausea and vomiting for one to three days is followed by a progressive decline in a child's mental alertness. Hospitalization in an intensive care unit is often required where medications and vital signs can be monitored. Intravenous medications are administered. Glucose and electrolyte solutions are given to correct blood chemistry levels. Surgery may be necessary to reduce the pressure on the brain caused by swelling. PREVENTION Never give aspirin to children under 18 years of age for any illness, especially viral infections. With a few exceptions, acetaminophen or ibuprofen can be given.

Reye's (pronounced "rye") syndrome is a disease which is believed to be caused by the ingestion of medicines (such as aspirin) that contain salicylate (pronounced "sa(LISS(a(late") that affects all organs of the body, but affects the liver and brain most lethally. It occurs when abnormal accumulations of fat develop in the liver and other organs of the body, along with a severe increase of pressure in the brain. It affects mostly children and teenagers and appears soon after flu-like infection or chicken pox. Reye's syndrome occurs most frequently during the months of January, February and March when influenza is most common, although cases have been reported throughout the year. Reports of the syndrome appeared as early as 1929, but it wasn't until 1963 that the symptoms were first recognized as a distinct disease by R. Douglas Reye, M.D., an Australian pathologist. Dr. Reye suspected that a drug or poison (later identified as aspirin) triggered the disease's development, but there was no proof of such a substance in the homes of the children who acquired the illness, nor did autopsies reveal the presence of any such substance ingested by the child.     Reye’s Syndrome Reye’s syndrome is a serious disease that affects many internal organs, particularly the brain and liver. It primarily strikes children between the ages of 4 and 15 years old. Many cases of Reye’s syndrome occur after a viral infection like the flu or chickenpox have occurred. Reye's has also been associated with Epstein-Barr virus, influenza B , and enteroviruses (viruses that infect the gastrointestinal tract). Four to six days after the beginning of the viral illness, the child suddenly develops a fever and severe vomiting. Other symptoms include: mental and personality changes that will appear as confusion, drowsiness, lethargy, memory lapses, and/or irritability and even unusual belligerence. The child may experience double vision, weakness and paralysis in the arms or legs, palpitations, speech impairment, impaired skin integrity, and hearing loss.     Definition: Inflammation of the cornea and conjunctiva caused by an
    adenovirus. It usually consists of a triad: pharyngitis, follicular

This could be an upper respiratory tract infection, diarrhea of infectious origin, or chicken pox (Reye's syndrome symptoms usually appear three to five days after the onset of the chicken pox rash). Many cases are mild and may even go undetected; others may be severe, requiring aggressive care. Symptoms include nausea, vomiting, lethargy and indifference. The child may exhibit irrational behavior or delirium, and rapid breathing. In the later stages, breathing becomes sluggish and the child becomes comatose, with dilated pupils. The liver may be enlarged, but there is usually no jaundice or fever. Reye's syndrome is rare: approximately 0.1 cases per 100,000 population. It is, however, often thought of when a child has continual vomiting or change of mental status - particularly after a recent viral illness. Although severity varies, Reye's syndrome should be considered an acute disorder that is life threatening. Description In 1963, a North Carolina doctor reported an epidemic of 16 fatal cases of an encephalitis-like illness during an influenza B outbreak. It became known as Reye's (pronounced rye's) syndrome. The syndrome had been reported as early as 1929 but now was identified and characterized as a distinct entity. In the late 1960s the Centers for Disease Control and Prevention developed case criteria that included mental status changes (delirium or coma) and a liver biopsy that showed fat accumulation in the liver (or high levels of liver enzymes and ammonia in the blood).
