It can happen from a variety of circumstances, Hirschprungs Disease, bowel malrotation, Chrohns Disease, Gastrochisis as well as other diseases or malformations. The problem with SBS is that with only a small portion of intestine, the GI tract is unable to absorb the nutrients as easily from food ingested or tube fed (enteral feeds). Necrotizing Enterocolitis and Short-Bowel Syndrome Necrotizing enterocolitis This acute gastrointestinal disorder was first described in the German literature in 1825 [1], but is still something of an enigma. Despite years of investigation, the etiology remains unclear, and accepted prevention and treatment strategies are lacking. Necrotizing enterocolitis (NEC) predominantly affects premature infants; less commonly (10%), term newborns, and, rarely, severely debilitated children. There have been instances of nursery epidemics. My name is Jim Combs, just so you will know I am a real person. Every thing I say is from my experience in the last ten years with Short Bowel Syndrome, in fact I have what is called Extreme Short Bowel Syndrome. I have about 12 inches of active small intestine and about 10 inches of small intestine that was really not useable, but was viable enough to use as a conduit and reconnect. This give the Doctors enough that I could be re-connected and not have an ostomy. I have about 22" total out of a possible 22 FT or so that is normal My small Intestine is made up of short pieces, mainly some of the "loops", sewn together, and according to the Doctors the longest single piece of intestine is "almost" 6" long. I have found that if I stretch while reaching to a top shelf, or have a "jar" by hitting a "chug hole" in the road, or even the shaking of a ridding lawnmower, it stretches the intestine enough that I have terrific pain. Other Companies Suzanne K. Thomas was born on 3/31/00. Stephanie had a perfect pregnancy and was going to deliver one month early. When the baby was born cesarean section the baby looked perfect and beautiful but the doctors were sucking out brownish/green fluid from her mouth. We asked if this was normal and the doctor said"yes, but not this much." The baby's tummy looked very bloated. The doctors continued to suck this bile or mconium from her mouth and stomach for over 24 hours. The doctors told us that she most likely had cystic fibrosis. She has a 90% chance of having cystic fibrosis they said. Turns out that 36 hours after she was born the staff decided to do a surgery to see what was wrong and they found that her small intestine was pinched off in utero and most of her small intestine had evaporated. They called this an atresia. What should have been 150 to 200 centimeters of small intestine was now 30 and was sown together in 4 places. The staff explained that nothing was certain. All you can do is wait and drip feed Suzanne through a g-tube and hope that her small intestine grows and adapts to her body. Interestingly, the exact mechanisms leading to intestinal adaptation are still not known. Clinical Snapshot In any event, pediatric patients with SBS are at risk for numerous complications. These babies have frequent episodes of diarrhea and fluid and electrolyte imbalances. Inadequate vitamin intake can cause cracked skin at the corners of their mouths and sore tongues from riboflavin deficiency, or bleeding because of a vitamin K deficiency. Anemia secondary to the problems of malnutrition may occur. And patients who require a central venous line (CVL) are at risk for developing life-threatening sepsis. Other chronic conditions include changes in bacterial intestinal flora, including bacterial overgrowth and parenteral nutrition-induced liver disease, which can be deadly. The most common cause of death in these children is, in fact, liver failure as a consequence of the parenteral nutrition required during the first years of treatment.
