Specific Diagnoses - Short Bowel Syndrome
Does anyone know of a support group for parents of children surviving necrotizing enterocolitis (NEC) with short bowel syndrome (SBS)? There does not appear to be a specific support group for children who had NEC as infants resulting in SBS. However, there is a foundation called the Oley Foundation which is national and is a support group for kids on long term TPN. Within this group, there are many children who have SBS resulting from NEC. The Oley foundation has a newsletter as well. The number for the Oley foundation is 1-800-776-OLEY. After calling this number, the family will be directed to the closest group in their region. Most of the families involved have children with SBS who had NEC as infants. There is an annual conference for the families where MD's and RD's present the newest research and treatments for sequlae from SBS. Management of short bowel syndrome
Loperamide should be given as capsules or as tablets because the sorbitol in the liquid causes diarrhoea, she said. The dose should be started at 4mg four times a day, increased every few days to a maximum of 16mg four times a day. It acts locally on opioid receptors in the gut and is not absorbed. Codeine can be used in combination with loperamide at a maximum dose of 120mg four times a day (starting at 60mg four times a day). Both drugs should be given 30–60 minutes before meals. There are systemic problems with codeine such as drowsiness and addiction. Co-pheotrope (Lomotil) should be avoided because of its atropine-like side effects. Octreotide is not licensed for use in small bowel syndrome but it reduces gastric, biliary and pancreatic secretions and slows gastrojejunal transit time, she said. A starting dose of 50–100µg should be used. Ocreotide is expensive and few general practitioners would pay for patients to have the drug. A similar drug, lanreotide, is a long acting, depot formulation. Proton pump inhibitors reduce gastric acid secretion which tends to be higher than normal in patients with short bowel syndrome. It is used in large doses: maximum of omeprazole 40mg twice a day or lansoprazole 30mg twice a day, although diarrhoea was more likely to occur with lansoprazole.

Irritable Bowel Syndrome
Irritable bowel syndrome (IBS) is very common, occurring in one out of every seven to ten people. It is the most common gastrointestinal problem seen by primary care physicians even though two-thirds of individuals with IBS never seek treatment. In the United States, more women than men are affected.
IBS is a disruption in the normal functioning of the intestines. Because no abnormality can be seen on x-rays nor detected by exams, it can lead to frustration on the part of both the physician and the patient. In spite of negative test results, it is important to understand that IBS is a real condition. Its symptoms are due to physiological changes in the intestine as it responds to diet, stress, and other influences.
Other intestinal or gynecological disorders as well as food allergies and digestive problems, can mimic IBS. It is important that you be evaluated by a physician. In more severe cases a gastroenterologist, a specialist in disorders of the digestive tract, should be consulted. He or she will take a thorough medical history and may perform a rectal exam; internal exam (sigmoidoscopy or colonoscopy); or stool, blood, urine, and other diagnostic procedures to rule out other causes.

Contributing Factors Nobody knows for sure what causes IBS. Despite extensive research, our knowledge of why bowel motility (how waste is moved through the intestinal tract) is altered in IBS is still incomplete. IBS can be affected by stress, activity, eating, and/or hormonal changes.
